Assist Life Stage Transitions is a vital part of the NDIS. This can be used as a disability support service to assist in transition and adaptation within your life stage.
Our Life Stage Transition program includes short and long-term support that focuses on assistance to live at home and community participation and helps you to coordinate your support.
